PATIENT COMMITMENT
MEMBERSHIP GUIDELINES
Eligibility Guidelines
Laser Vision correction patients must meet the following criteria to be eligible for the Patient Commitment program:
1. The patient must have their initial treatment at Weston's Las Olas Laser Eye Center for correction of myopia, myopia with astigmatism, low hyperopia or low hyperopia with astigmatism and have a myopic spherical equivalent of -10.00 D or less, astigmatism of no greater than -3.00 D and/or hyperopia of no greater that +3.00 at the time of their first treatment.
2. The patient must have followed the complete course of post-operative treatment required during the first 12 months after treatment, including post-operative follow-up visits, as prescribed by the surgeon and/or the affiliated eye doctor.
3. Following completion of the post-operative treatment period of 12 months, the patient must have annual eye exams with an affiliated doctor and document each visit. The affiliated doctor should fax any and all exams to the center and these will be kept in the patient’s files. Fees for annual exams are determined by the patient’s doctor and are the sole responsibility of the patient; the Weston's Las Olas Laser Eye Center Patient Commitment program does not cover the cost of annual eye exams.
4. The program does not cover the cost for a patient’s post-operative care after re-treatment unless this care occurs within the first year following the patient’s initial procedure. The cost is determined by the affiliated doctor and is paid to the doctor by the patient.
5. The ultimate decision as to whether or not a patient should be re-treated is made jointly by the surgeon, the patient and if applicable the affiliated doctor as long as a re-treatment is medically advisable.
6. A charge may be applied, if new technology becomes available, for any future procedures or re-treatments using that technology.
Exclusion Guidelines
Although we expect 99% of our patients to be eligible for the Patient Commitment program, there are exceptions or conditions under which further refractive surgery is ill-advised. These Patients, as described below, will not be eligible for the Patient Commitment program.
Exclusions (All TruVision Patients Excluded)
1. Patients with pre-operative degrees of myopia greater than -10.00 diopters (spherical equivalent), astigmatism greater than -3.00 diopters or hyperopia greater than +3.00 diopters.
2. Patients whose loss of vision is a result of an accident involving trauma to the eye.
3. Patients with diseases such as diabetes, active autoimmune and collagen vascular disorders.
4. Patients whose cornea is too thin as a result of previous surgery.
5. Patients whose vision is reduced due to irregular astigmatism.
6. Patients whose initial surgery was not performed at Weston's Las Olas Laser Eye Center.
7. Patients who had RK, ALK, or other refractive surgery, prior to having laser vision correction.
8. Patients diagnosed with an ocular disease such as cataracts, glaucoma, diabetic retinopathy, retinal tear, detachment, or macular degeneration.
9. Patients seeking a re-treatment as a result of developing presbyopia, even though presbyopia is a result of natural changes in vision.
10. Patients electing monovision treatment will only be covered in the eye treated for distance, as long as that treatment meets the eligibility requirements.
11. Travel costs, if any, are the patient’s responsibility.
12. Patients who fail to comply with the Patient Commitment rules and regulations.
13. Patients who fail to disclose pregnancy at the time of the initial surgery or who become pregnant during the first 6 months post-operatively.
